The nurse’s role in the school setting is multi-faceted. We work very closely with the students, parents, and staff to ensure the health and safety of all students. The nurse is responsible for maintaining the health records, including the immunization records and the State Mandated Physicals. These physicals are required for new entrants, including Circle of Friends, Kindergarten and again in 6th and 9th grade. Annual physicals are also required to participate in sports.

The nurse also attends to the illness/injuries of the students on a daily basis. Please contact your schools nurse if your child has a specific medical diagnosis, especially if he/she requires special accommodations and/or medication.
Authorization For The Administration of Medicines By School Personnel. If your child requires medication in school, this form needs to be completed by both the parent/guardian and the physician. This requirement includes both prescription medicine and over the counter medications (i.e. Advil, Tylenol). Your child may “self-carry” ONLY, inhalers and epi-pens, if specified on the order. These forms need to be completed for each school year.
If your child is out ill, please contact the school. If they are in Rockwell, Berry or Johnson School call the school nurse listed below. If they are in Middle or the High School, contact the attendance line. Please remember, do not send your child sick to school for the health and safety of all students.

Allergy treatment protocol form needs to be completed by physician and parent for anyone requiring medication for allergies.
Reminder to all students playing a sport, you will need a current physical dated within thirteen months, to participate. This must be completed before you can practice with the team. Also, the green form titled Athletic Emergency Form, which is located on the district BHS web- site under athletics, needs to be completed.
